While diverse in structure, culture and values, health systems worldwide share the challenge of addressing the needs of patients with complex chronic health and social care needs. A small, yet growing, population, these individuals expose the gaps in health systems and demonstrate where we can do better. Many of these gaps can be addressed through the adoption of both innovative and legacy technologies. In this talk, Dr. Steele Gray will discuss how the challenge of complexity can be addressed in primary care settings through digital technologies. Two digitally enabled primary care models will be discussed. The first is how digital technologies have been used to support integrated care models across in three different jurisdictions (Ontario, Quebec and New Zealand). The second is how technology can enable goal-oriented models, specifically looking at the electronic Patient Reported Outcome (ePRO) tool, a mobile app and portal systems co-designed by Dr. Steele Gray’s team along with patients and providers to enable person-centred goal-oriented care in primary care clinics. The role of careful user-driven development, pragmatic and real-world evaluation, and the adoption of an implementation science approach are critical factors to ensure technologies address system gaps, and can be feasibly adopted to improve care for patients with complex care needs. The discussion will end with a focus on the ongoing challenges of scale-up, spread and sustainability when considering adopting technologies in the health system and reflections on what is needed to move towards a digitized health system.

Dr. Carolyn Steele Gray is a Scientist at the Bridgepoint Collaboratory in Research and Innovation in the Lunenfeld-Tanenbaum Research Institute, and an Assistant Professor in the Institute of Health Policy, Management and Evaluation at the University of Toronto. Dr. Steele Gray’s program of work focuses on the role of health information technology in supporting patient-centred care delivery for patients with complex care needs. Adopting an implementation science approach, Dr. Steele Gray explores how technology can be leveraged to enable implementation and sustainability of innovative models of care as a means to support health system transformation. As part of this work she has been developing and evaluating new technologies, most notably the electronic Patient Reported Outcome (ePRO) mobile application and portal, building change management and implementation frameworks to support implementation of new and existing technologies, and studying technology adoption locally and globally.
