Quynh Pham, PhD

Canada’s demographics are shifting towards an aging, ethnoculturally diverse, and chronically ill population. The Covid-19 crisis has escalated the novelty of virtual care into a necessity for receiving any care. However, virtual care products and services have long overlooked the needs of minority communities. For many individuals from minority communities living with chronic disease, the prevailing notion of ‘self’ management is a misnomer since their care is actually shared with their family. Family caregivers are an integral part of a patient’s care team and can bridge the gap between the support patients need and what virtual care can provide. Many patients with an active and involved family caregiver have better self-care practices and health outcomes than patients who manage their chronic disease in isolation. This seminar will explore how family-centered virtual care for chronic disease management can improve the health of minority communities.
