Integrated Health service Delivery is the principle Goal of Health Transformation now underway in Ontario. Bringing Primary Health Care into this tent with other key Health Service Providers such as Hospitals, Long Term Care, & Home Care is one of the key deliverables.
The face of these integration efforts is focused on the creation and operalization of Ontario Health Teams. Many have tried in the past to gather the many players in Primary Care together but there are few if any success stories.
For the occasion, we have assembled a panel with 3 different Practitioner (Manager/Leader) perspectives to discuss this challenge. One represents a Community Hospital, another a Community Health Center and the last one might best be described as a privately managed comprehensive Primary Care practice.
They will be coordinated in their discussion by one of Telfer's aspiring Faculty whose area of research is Primary Care, PhD Agnes Grudniewicz, Assistant Professor.
